Become a Member

At Florida Gulf Coast PVA, we are fully committed to supporting veterans with spinal injury or disease and connecting them with the appropriate resources and publications to best meet their needs. Individuals who meet the following criteria are eligible to become a member and join the PVA team:

  • United States citizen
  • U.S. Armed Forces veteran whose discharge was other than dishonorable
  • An individual who has suffered a spinal cord injury as a result of trauma or disease (Note: The injury or disease onset could have occurred after discharge/retirement from military service.)

You may qualify for membership if you have any of the following spinal diseases:

  • Amyotrophic lateral sclerosis (ALS)
  • Lupus
  • Tuberculosis
  • Multiple sclerosis (MS)
  • Poliomyelitis
  • Spina bifida
  • Spinal stenosis
  • Tumors of the spinal cord

Those whose condition is not listed above may still qualify for PVA membership and should contact the Gulf Coast Chapter office for assistance. If you meet the eligibility requirements to join PVA, mail the completed PVA application to:

PVA Membership Application

Please note that applications will only be processed with DD214 forms included. Those who do not meet the qualifications for PVA membership can join the associate membership program or become a PVA volunteer. These programs enable individuals with a deep interest in PVA to contribute personally and/or monetarily.
